Neami outreach provides individual support and rehabilitation to people in their own homes or local community settings. Â Individual outreach support aims to provide people with living skills, whilst encouraging and creating connections to their local community. The service is flexible and encourages people to identify their own areas of need.
Individual outreach assists consumers to:
- Develop social, recreational and living skills
- Link into community based activities and services
- Receive assistance with education, training and employment
Neami provides home based outreach through a psychosocial rehabilitation framework, with each activity aimed at increasing consumers’ independence and social connectedness.
