Neami home based outreach provides individual support and rehabilitation to people in their own homes who have difficulty accessing services provided by the community.  Home based outreach aims to provide people with living skills in their own homes, whilst encouraging and creating connections to their local community.  The service is flexible and encourages people to identify their own areas of need.

Home based outreach assists consumers to:

• Develop social, recreational and living skills
• Link into community based activities and services
• Receive assistance with education, training and employment

Neami provides home based outreach through a psychosocial rehabilitation framework, with each activity aimed at increasing Consumers independence and social connectedness.
